Specimen Submission Guidelines

  1. Specimen Documentation

    The delivery of exceptional hematopathology services, whether for initial diagnosis or management, is founded not only on an implicit assumption of quality and accuracy, but a partnership between the ordering physician and the hematopathologist that allows for immediate consultation and thoughtful dialogue.

    To ensure that Miraca Life Sciences provides an unmatched level of quality and accuracy, the company asks that each patient sample submitted include:

    • Current office CBC
    • Clinical history or patient note
    • Reticulocyte count and prior therapy if relevant

    Additionally, proper completion of the Miraca Life Sciences Hematology/Oncology Test Requisition will allow us to process your patient samples without the inconvenience of calls to your office and staff. When completing the requisition, please provide the information below as required by CLIA:

    • Patient's full name
    • Date of birth
    • Sex
    • Specimen type — peripheral blood, bone marrow aspirate, etc.
    • Date and time of specimen collection

    Also, please provide a copy of the patient's insurance card or billing information. The Miraca Life Sciences Comprehensive Diagnostic Analysis (bone marrow or blood) is considered a request for all medically necessary technologies required to provide appropriate information. We are committed to proper utilization of testing and keeps responsible ordering at the forefront of company efforts.

    A request for a Limited Diagnostic Analysis will include a Clinical Pathology Consultation.

  2. Specimen Collection

    To provide the best possible blood or bone marrow analysis, we ask that your office follow the steps below:

    1. Label all containers and slides with the patient's name and date.
    2. Submit bone marrow core and clot in formalin provided and indicate anatomic location (e.g., right side, bilateral). Place both core and/or clot vials in biohazard bag and ensure bag is tightly sealed to prevent formalin fume exposure.
    3. If you require RPMI in anticipation of a dry tap, please contact Miraca Life Sciences Client Services. Saline is also an acceptable substitute.
    4. Submit aspirate or blood smears as indicated on the Miraca Life Sciences Optimal Specimen Requirements. Ensure that slides are completely air-dry before placing in the slide cassette.
    5. If necessary, we will produce smears from material provided. However, fresh bedside smears are preferred.
    6. For molecular studies, provide 1 purple-top (EDTA) tube containing 2–3 ml of bone marrow aspirate or peripheral blood.
    7. For cytogenetics and/or FISH studies, provide 1 green-top (sodium heparin) tube containing 2–3 ml of bone marrow aspirate or peripheral blood.
    8. For flow studies, 1 green-top (sodium heparin) tube is preferred or 1 purple-top (EDTA) tube containing 2–3 ml of bone marrow aspirate or peripheral blood.
    9. Provide patient clinical history, CBC and reticulocyte count, if relevant.
    10. Complete the Miraca Life Sciences Hematology/Oncology Test Requisition Form.
    11. Call Client Services for specimen pickup. Specimens should be shipped within 24 hours of draw and stored at room temperature. Please insert a FROZEN cold pack for transport.

  3. Specimen Preparation and Shipping

    Hematology Shipping instructions

    1. Complete the requisition form. Be sure to include the patient history and billing information (especially ICD-9 codes).
    2. Label each tube/slide/vial with the patient's name, date of birth and medical record number (MR#).
    3. Place collected sample into clear biohazard bag and seal (ensure formalin vial lids are tightly secure, then placed in the biohazard bag). Place formalin vial and plastic slide cassettes into the foam insert (please allow the slides to completely dry before placing into plastic cassette). Place the tubes next to the foam insert.
    4. Place the supplied cardboard insert on top of the specimens and place frozen pack on top of the cardboard. Do not allow the frozen pack to come into contact with the specimen.
    5. Ensure all components fit snugly in the Styrofoam container and place the lid on the cooler.
    6. Place the top copy of the completed requisition, patient face sheet and clinical history on top of the Styrofoam cooler and close the shipper kit.
    7. Insert shipper box into the supplied LabPak™ and seal.
    8. Attach the appropriate (UPS or FEDEX) shipping label. All specimens are shipped Priority Overnight/Next Day Air and will be received within 24 hours of draw/procurement.
    9. Contact Client Services to arrange specimen pickup at 855.856.0656.

    Peripheral Blood Shipping instructions

    1. Complete the requisition form. Be sure to include the patient history and billing information (especially ICD-9 codes).
    2. Label each tube and slide with the patient's name, date of birth and medical record number (MR#).
    3. Place blood collection tubes/slides into foam insert and seal foam insert in clear biohazard bag. (Let slide dry completely before placing into plastic cassettes.)
    4. Insert sealed biohazard bag with specimen(s) into the insulated foil sleeve along with the cardboard insert and frozen cold pack. Place the cardboard sleeve between specimen bag and frozen cold pack — do not allow the frozen pack to come into contact with the specimen.
    5. Place insulated foil sleeve into the shipper along with the top copy of the completed requisition, patient face sheet and clinical history.
    6. Insert shipper box into the supplied lab pak and seal.
    7. Attach the appropriate (UPS or FEDEX) shipping label. All specimens are shipped Priority Overnight/Next Day Air and will be received within 24 hours of draw/procurement.
    8. Contact Client Services to arrange specimen pickup at 855.856.0656.

    Specimen Transport

    Miraca Life Sciences has arranged special transportation agreements with FedEx and UPS to ensure rapid and safe shipping of specimens. To ensure the viability of the specimen and the timeliness of the assay, all specimens are shipped Priority Overnight/Next Day Air, including Saturday delivery. Our client services team works with these transportation companies on a daily basis to make certain our clients have the supplies they need to ship specimens and that shipped specimens are delivered on time.

    Specimen pickup is scheduled through Client Services in order to ensure the timely delivery of specimens. After hours and weekend transportation are available. Contact Client Services at 855.856.0656 to arrange pick up of your specimens.

    Please note: In order to maintain the specimen viability, it is important to contact Client Services for every shipment so our staff can track the specimen and resolve any potential shipping issues.

    Shipper Kits

    Miraca Life Sciences provides shipper kits at no charge to the client. These kits are designed exclusively for Miraca Life Sciences and are able to withstand extreme temperatures, ensuring each specimen remains at the optimal temperature for specialized testing. Our kits are developed in accordance with Packaging Instruction 650 as outlined in the Dangerous Goods Regulations, a publication provided by the IATA (International Air Transportation Association) organization.

    Depending on specimen and collection requirements, several different shipper kits are available. When you receive the shipper kits from us, be sure to remove the cold packs and place them in the freezer/refrigerator so they will be ready for specimen collection.
